135dc73077 Bug 467833 - Wait Shell initialization and writer thread leak 
- Introduce RSE_SHELL_READY_PING to optionally ping the target every
  specified milliseconds after connect (maximum 10 times), until an
  "echo" command actually gets executed.
- Fix a shell writer thread leak

21e5739a8f Rework fix for bug 420928 
We need to continue polling the text model until the terminal is
actually disconnected from the input stream, otherwise error messages
about connect failures don't appear in the terminal.
